Facial Dermatofibrosarcoma Protuberans: Atypical Location – Case Report

Abstract

Introduction: Dermatofibrosarcoma protuberans is a malignant soft tissue tumor affecting the dermis, subcutaneous tissue, muscle or fascia. Its occurrence in the head and neck is uncommon (=6%). It has a mesenchymal origin and may be associated with previous trauma and genetic or environmental factors. Although malignant, it shows low metastatic rates and high rate of local recurrence, thus requiring surgical management. Case report: We present the case of a 47-year-old patient with a facial mass of 3-year evolution, showing slow growth and measuring 10 cm in diameter at the right submandibular angle. The patient underwent surgical resection with intrao­perative frozen section biopsy, confirming negative margins. The resulting defect was reconstructed with a full-thickness myofasciocutaneous pectoral flap, with op­timal postoperative recovery. At 16 months of follow-up, there was no recurrence, and esthetic results were satisfactory. Conclusion: This case highlights the rarity and successful management of facial dermatofibrosarcoma.
